PRISMHealth: The Program partners with Livongo and Hinge Health to empower your employees with real-time digital solutions for diabetes and musculoskeletal health management!

Wearable Health Solutions

Employees are provided a digital glucometer (Livongo) or a wearable sensor (Hinge) and results are collected via mobile app and sent to a clinical care team. Coaching and in-person support is provided to help the employee continue on a path to health and wellness.
