| Friendly reminder: all applicable forms for the 2024/2025 PRISM Property Program renewal were due November 1, 2023. The updated information you submitted will determine your renewal premium. Any changes reported after November 1, 2023 may need to be endorsed onto the policy and/or require additional premium. Please refer to the PRISM Renewal Timeline Policy for important deadlines. | Register today for the upcoming virtual training: Cal/OSHA Form 300, 300A, and Serious Injury Reporting Procedures. On November 8, from 9 to 10:30 am, PRISM’s Juan Cajandig, Sr. Risk Control Specialist will teach supervisors and employees the criteria for reporting serious injuries, maintaining the Cal/OSHA Form 300, and completing the Cal/OSHA Form 300A.
