The PRISM Executive Committee discussed data collection issues at their April 30, 2004 meeting, and authorized staff to:

• Request permission from the members to obtain loss data directly from their Third Party Administrators (TPAs)
• Submit loss data for statistical analysis upon request of related organizations

PRISM is asking each member to provide authorization to move forward in this endeavor. 

Please complete the Authorization to Obtain Loss Data Form and return it to PRISM. For JPAs whose members use multiple TPAs, please forward the background information and Authorization Form so your members can extend their authorization.
