The U.S. Department of Transportation (DOT) has announced a revision to the Custody and Control Form (CCF) used for drug and alcohol testing.
Upon review of the new form, you will find the most noticeable change is the addition of a field for the donor’s commercial driver license (CDL) information. The new CCF can be used now, although it isn’t required until September 1, 2021. In the meantime it is recommended to use up any remaining forms you have on hand, and make the switch once those supplies are gone.
